In order to have an efficient consultation with one of Highmark Direct's licensed associates, please bring the following with you to your appointment:

1. A recent physical or medical consultation within the past five years.  Be sure to have:
   • Date of last visit
   • Doctor’s name
   • Reason for visit
2. Drivers license or Photo ID for verification purposes
3. Social Security number for all individuals applying
4. Depending on how you are applying for your policy you have a number of payment options.  If you are applying by paper application you may pay with a check, cash, or money order.  If you are applying electronically, you may pay with a credit card.
5. Insurance card (if available)
6. One month’s proof of house hold income for individuals applying for an income-based product
7. Birth Certificate or Passport for children applying for CHIP
8. A list of medical conditions (Diabetes, heart disease, etc.) along with details regarding treatment:
   • Most recent test results -- Special readings, scans, etc.
   • All medicine taking:
   • Name
   • Dosage
