What To Bring

Be prepared when you meet one-on-one with a health insurance store associate. See a list of all the documents necessary to have an efficient consultation.

In order to have an efficient consultation with one of Highmark Direct health insurance store's licensed associates, please bring the following items with you to your appointment:

If you are an Individual/Non-Medicare:

  1. Names, birthdates, and Social Security numbers (or document numbers for legal immigrants) for everyone in your family who needs health coverage
  2. Employer and income information (e.g., pay stubs, W-2 forms — Wage and Tax Statements)
  3. Health insurance policy numbers for anyone in your family who currently has health coverage
  4. If losing group coverage, a letter showing proof of cancelation
  5. Valid email address if registering on at healthcare.gov

If you are Medicare eligible:

  1. Your red, white and blue Medicare Card
  2. Retirement Month/Year, if applicable
  3. Name of Primary Care Provider
  4. Current Health Insurance of Medicare Plan Name and Policy Number
  5. Medicaid Number, if applicable
  6. If you have access to a printer, a completed “Scope of Sales Appointment Confirmation Form”.
